MAXIMUS

Developmental Disabilities Home and Community Based Services (DD/HCBS) Waiver and Consumer Directed Care Plus (CDC+) Waiver:

1. Cost plans that are $80,275 and over

2. Cost plans that contain residential habilitation in a residential setting

3. Amendments to any cost plans where any services on the cost plan meet one or more of the criteria above

PLEASE NOTE: IF A COST PLAN IS AMENDED AND NO LONGER INCLUDES RESIDENTIAL HABILITATION IN A RESIDENTIAL SETTING AND/OR BECOMES LESS THAN $80,275, IT SHOULD BE SENT TO APS HEALTHCARE, REGARDLESS OF WHETHER MAXIMUS COMPLETED A PRIOR REVIEW.

 

APS Healthcare

Developmental Disabilities Home and Community Based Services (DD/HCBS) Waiver, Family and Support Living (FSL) Waiver, and Consumer Directed Care Plus (CDC+) Waiver:

1. Cost plans that are under $80,275 and do not include residential habilitation services in a residential setting (Cost plans that do not meet criteria for MAXIMUS review)

2. Amendments to any cost plans under $80,275 that do not include residential habilitation services in a residential setting (Cost plans that do not meet criteria for MAXIMUS review)

PLEASE NOTE: IF A COST PLAN IS AMENDED AND NOW INCLUDES RESIDENTIAL HABILITATION IN A RESIDENTIAL SETTING AND/OR BECOMES $80,275 OR MORE, IT SHOULD BE SENT TO MAXIMUS, REGARDLESS OF WHETHER APS HEALTHCARE COMPLETED A PRIOR REVIEW.

 

Area APD Offices

1. Emergency requests where an individual's health and safety is in jeopardy without the immediate provision of a waiver service (please refer to the APD policy entitled, "Emergency Waiver Services and Cost Plan Approvals made by Area APD Offices")

2. Amendments to cost plans for provider changes where the service, and the intensity, frequency, and duration of the service does not change

3. Rate changes if the intensity, frequency, and duration of the service(s) does not change, i.e., provider changes from solo to agency and the rate differs, but service provision does not.

4. Supported Living Stipends

5. General Revenue or IFS funding approvals

2008 PSA Training
The Florida MAXIMUS Prior Service Authorization Program completed 15 training sessions in the Fall of 2007 for Waiver Support Coordinators (WSC) and Area Agency for Persons with Disabilities (APD) staff. Recent changes in law implemented by APD resulted in transitioning of all PSA submission responsibilities from WSC to Area APD staff.

Designated Area APD staff now have the ability to submit Prior Service Authorization requests on-line. Additional Area APD staff needing to gain access to the web application should submit a MAXIMUS PSA Web Access Form (found below) and refer to the Training Materials (also attached below). Once the form is completed and signed by the staff and his/her supervisor, it may be faxed to the PSA Unit at: (866) 887-0743 (Areas 1, 2, 3, 4, 12, 13, 23) or (866) 407-7301 (Areas 7, 8, 9, 10, 11, 14, 15).
